The game introduces a new narrative focusing on a character named Aidan. His girlfriend, Sunny, has been pulled into the otherworldly hellscape known as the Labyrinth by the dreaded Cenobites. To save her, Aidan must wield the power of a mysterious puzzle box—the Genesis Configuration—a new iteration of the iconic Lament Configuration.
The Story: Aidan must use the power of the Genesis Configuration to navigate Hell and escape the Cenobites’ clutches, unwittingly kickstarting Pinhead’s ritual to unleash greater torment upon the world.
The Gameplay: Advertised as a first-person action horror survival experience, players will combine earthly weapons with the hellish, unique abilities granted by the Configuration to fight against a terrifying array of enemies: hell’s wretches, deviants, cultists, and infernal priests.
The Atmosphere: Early previews from Gamescom 2025 have already cemented the game’s reputation for extreme, uncompromising gore and “mature” themes, with some reports even mentioning that developers had “sick bags at the ready.” This is clearly a title that aims to push the rating system to its absolute limits, staying true to Barker’s vision of sadomasochistic horror.
